40 Plus Health Checks
Everyone aged 40-74 who does not have a pre-existing heart condition is eligible for a health check once every five years. It is a free check-up of your overall health. It can tell you whether you’re at higher risk of getting certain health problems, such as:
During the check-up you’ll discuss how to reduce your risk of these conditions.

Annual health check Learning Disability and Autism
All patients aged 14 years and over with a learning disability and/or autism are eligible for an annual health check. An annual health check can help you stay well by talking to a doctor or nurse about your health and finding any problems early, so they can be sorted out.
You do not have to be ill to have a health check – in fact, most people have their annual health check when they are feeling well.
If you have a learning disability or care for somebody who does, make sure we are aware of this because not everyone may be on the practice’s Learning Disability register. You can do this by telling our reception team, doctor or nurse.
Once on the register you will receive an invite each year to attend an appointment.

Carers health checks
The carers health check is the same as the NHS Health Check but is about looking after you as a Carer. It will focus on your own health and wellbeing and whether there are any areas you need support with. It is a chance to talk about your concerns and hopes for the future. The check covers the questions which Carers have told us are important.

Our practice Armed Forces Champion is Paula.
Our Armed Forces Champion will offer advice and support and will be able to signpost our Military community to the services they may require.
Please let the practice know if you have served in the Armed Forces as a Regular, Reservist or Adult Volunteer Cadet. We have a pack that you can collect from our reception.
The practice works with Sunderland CCG Armed Forces Leads and Veterans in Crisis Sunderland to ensure those who have served or currently serving in the Military have access to information across the city on services and support that is available.

Details of over 2000 self help groups, patient groups and similar organisations, fully cross referenced and details checked annually.

Gives patients and the public information about the NHS locally and nationally and includes an overview of how the NHS works.
The site holds information on each local family practice, pharmacy, optician and dentist surgery as well as NHS organisations.
Each PCT already has a web editor whose role is to input information about all services provided in their region. You can also gain access to our NHS website.
This is a national electronic referral service giving patients a choice of place, date and time for their first outpatient appointment in a hospital or clinic.
You can choose the hospital or clinic, and then book your appointment to see a specialist with a member of the practice team at the GP surgery, or at home by telephone or over the internet at a time more convenient to them.

If a GP or another member of our health care team believes you need hospital treatment or specialist care elsewhere, they will ask you where and when you would like to go.
They can then refer you electronically while you wait.

Practice Nurse gives children their immunisations.
Doctors carry out Postnatal checks during this clinic. It is important that you attend with your child for all of their immunisation.
Antenatal Clinic – Monday 1.30 – 3.30pm; the Midwife sees patients by appointment.
The Practice provides:
- Oral Contraceptive
- Contaceptive Injections
- Fitting Contraceptive impants and coils
- Emergency Contraception(please arrange to see/talk to a GP as soon as possible if you require this)
All doctors are able to give advice in all of the above methods of contraception.
Patients who have been started on the contraceptive pill or injectable contraception can attend the practice nurse for their yearly check-up. The doctors are also happy to give advice to those patients preparing or wishing to become pregnant.
