You must notify us of any changes in your personal details so that we can amend our records accordingly. Change of Name can only be amended on proof of details by either a marriage certificate, legal document.
You must also inform the Contractor Services Agency of any change of name or address, by sending them your medical card. It is most important that we have your up to date contact details.
