Registering With Us

The practice operates a no discrimination policy.

We are welcoming new patients to register with the practice that live within our catchment area which covers SR1, SR2, SR3 & SR4.

On joining the practice you will be registered for administrative purposes with one of the GP’s. However as patient you do have the right to express a GP preference when making an appointment, it is in your interest to get to know one specific doctor and stay with that doctor during any one episode of illness.

The practice will endeavour to comply your request, however it may not always be possible if this is the case an explanation will be given.

We aim to deliver a personal style of care and hope as a patient you will help us to fulfil our role and responsibilities to you. We work very much as a team with nurses taking on many tasks which were previously only undertaken by doctors. The doctors will still retain overall responsibility for patient care.

When registering you will be asked to complete a new patient registration form (GMS1) and new patient questionnaire, this will ask you your name, address, date of birth, previous name, address and the name and address of your previous GP.

You will be given an appointment with the Health Care Assistant for a simple health check, during which we will take details of your medical history and current medication.
